Professional Liability Waiver & Release

Participant Acknowledgment & Assumption of Risk
I, the undersigned, understand that participation in Pilates classes, workshops, and related fitness activities (the “Activities”), whether held at Pilates of Charleston’s studio, online, or at any off-site location, involves physical movement and carries inherent risks of injury. I voluntarily choose to participate in these Activities with full knowledge of these risks.

Health Disclosure
I confirm that I am physically fit to participate in Pilates and related fitness training. I agree to notify Pilates of Charleston and its instructors of any medical conditions, injuries, or limitations that may affect my participation. I understand that Pilates of Charleston and its instructors are not medical professionals and cannot diagnose or treat medical conditions.

Release of Liability
In consideration of being allowed to participate, I, on behalf of myself, my heirs, executors, and assigns, hereby release, waive, and discharge Pilates of Charleston, its owners, instructors, employees, and contractors (collectively, “Released Parties”) from any and all claims, demands, or causes of action arising out of or related to my participation in the Activities, including but not limited to injury, illness, disability, or death, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Indemnification
I agree to indemnify and hold harmless the Released Parties from any liability, damages, costs, or expenses (including attorney’s fees) incurred as a result of my participation in the Activities.

Use of Facilities & Off-Site Locations
I understand that this waiver applies regardless of whether the Activities take place at Pilates of Charleston’s studio, in a private or public off-site location, or through online instruction. I agree to follow all rules, guidelines, and instructions provided by instructors to ensure safety during participation.

Media Release (Optional)
I grant permission for Pilates of Charleston to use photos, videos, or recordings of classes that may include my likeness for marketing, educational, or promotional purposes.

Acknowledgment of Understanding

By purchasing or reserving a class, package, or membership with Pilates of Charleston, I acknowledge that I have read, understood, and agreed to the terms of this Liability Waiver & Release. My participation in any Pilates of Charleston class—whether onsite, offsite, or online—constitutes my acceptance of these terms.
